Fasting insulin vs HbA1c: what each test tells you

These markers describe different aspects of glucose regulation. One cannot be converted into the other.

MarkerWhat is measuredRole on this site
Fasting glucoseGlucose concentration at samplingAn input to fasting indices
Fasting insulinInsulin concentration at samplingPaired with glucose for HOMA1 or QUICKI
HbA1cGlycated hemoglobinUsed to estimate average glucose with ADAG

Fasting insulin captures a sampling moment

An insulin assay measures circulating insulin under the conditions in which the blood was collected. Preparation and medication history matter when interpreting it.

HbA1c gives a longer view

HbA1c reflects glucose exposure over roughly three months. It does not describe every high or low reading, and conditions affecting red blood cells can alter it.

Why the calculators give different kinds of answers

HOMA1 uses paired fasting glucose and insulin. The eAG calculator uses HbA1c. Feeding a single fasting glucose value into the reverse eAG equation does not turn it into a measured HbA1c.

Which test should you request?

The choice depends on the assessment. Review it with your clinician; this site does not recommend an insulin test as a replacement for established diabetes diagnostic tests.

Common follow-up questions

Can I estimate HbA1c from one fasting glucose result?

No reliable individual estimate follows from a single fasting reading. The reverse ADAG equation expects representative average glucose over time.

Is a fasting insulin test the same as an HbA1c test?

No. Insulin testing measures the hormone in a blood sample; HbA1c reflects glucose exposure over time. They answer different questions.

Does HbA1c show today’s glucose level?

HbA1c reflects glucose exposure over approximately three months. It cannot show the timing or size of a particular glucose spike.
